There are seven questions that include a 200-word Executive Summary of the nominee’s achievements and a 2,000-word nomination.

In your write-up, you must include evidential material, such as financial details, data and customer feedback, and you can include links to relevant documents. You can also upload two additional supporting documents, such as media articles, letters of endorsement and photographs
